Project Proposal Description

Briefly describe the project and the community need to be addressed.

Answer Volunteer Navigators connect individuals with community resources to address social needs to enhance their health using a social care platform called caringforcommunities.findhelp.com

What is the mission of the agency or organization?

Answer Individuals are screened during health care visits to identify areas of need, such as food, transportation, clothing and utilities. Our Community Connections Team (CCT) enlists UW volunteers to connect individuals with local resources to meet their needs and helps individuals and families improve their health and quality of life.

What skills are required for this project? 

Answer Navigators connect with individuals and provide them with local resources to meet their social needs to improve their health and quality of life.

4.

What are the objectives for the student learning and service?

Answer You will gain valuable communication skills, critical thinking, community resource awareness and a deeper understanding of the broader side of healthcare.

5.

What are the availability requirements for the student?

Answer A three-hour shift, one day a week for 2 semesters (onboarding and training provided).
